Community Health Connect brings healthcare directly into the neighborhoods we serve through welcoming, family-friendly community experiences designed to make healthcare more approachable, transparent, and accessible.
What it is
Conversations, not booths.
Community Health Connect is Safe Access Network's flagship community engagement program. Unlike traditional health fairs, it is built around conversations, not booths. Residents meet trusted local providers and organizations in a welcoming neighborhood setting, and those first conversations grow into lasting connections to care.

Why we created it
Healthcare can be overwhelming.
Many people do not know where to begin, which provider they need, what community resources exist, how to access care, or who they can trust.
At the same time, excellent providers and nonprofits struggle to genuinely connect with the communities they serve. Community Health Connect bridges that gap.
